- An initial personal consultation with Client and representatives will be arranged to discuss all expectations regarding care needs including personal care, social and financial needs.
- Explain the basic fees & charges that incur when residing in an aged care facility and if necessary we can provide referrals to qualified Aged Care Financial Advisors for further assistance
- Provide a general overview of the care and services a resident should receive will in care
- Assistance in sourcing all relevant assessments for approval for permanent residential aged care
- Assist by finding and securing suitable care in an aged care facility including touring facilities in your preferred locations
- Arrange the admission documentation and entry into your chosen aged care facility

- Transitional Case Management post Residential Aged Care Placement – 12 week service

- Transitional care management provides support to the client/representative after placement in aged care
- Supports the client in settling into care with regular visits from our consultant
- Reviews with clinical care team including care planning during first month of care
- Arrange a Case Conference with Client/ Representative and Care Team at the end of 4 – 6 weeks post admission
- Review outcomes of Case Conference over the next 4-6 weeks
